This is the 2024 NASD Online Scheduler & Benefits Portal. We encourage you and your family to become familiar with this website. Here, you will find your insurance benefits, claim forms, links to the providers, and a link to the enrollment platform to view your benefit confirmation and update beneficiaries.

Scheduling Instructions:
1. Select an available date on the calendar and then click on an available time to select your preferred appointment.
2. Visit nasd.mybenefitsinfo.com/ to review the Benefit Overview before your appointment.
3. SSN Required for NEW Dependents (spouse or children).

User ID: Your Social Security Number
Password (PIN): The last 4 digits of your Social Security Number followed by the last 2 digits of your birth year.
Important:
- After completing the Enrollment, you cannot change your benefit elections.
- You can view/print the Benefits Confirmation sheet and contact Emma Clancy in HR via email at [email protected] with any changes, questions, or discrepancies you see.
NIS Evidence of Insurability Online Form
The EOI for the Voluntary Long-Term Disability Buy-Up coverage is required for the National Insurance Services (NIS). Your coverage will drop if the EOI form is not submitted within 60 days after completing your enrollment process.
